Care Consulting
Care Consulting offers personalized care planning and vital referral services for aging adults (60 years or older) or those living with a disability, as well as the family members and friends who care for them.
This service is designed to help you proactively identify and access the support needed to age well and independently in your own home for as long as possible.
WHAT TO EXPECT
A trained Care Consultant will spend 1-2 hours in your home to conduct a comprehensive assessment. During this confidential visit, we'll discuss your current needs, challenges, and goals.
The consultant will then provide:
Customized Care Planning: We help you map out a strategy for care by reviewing your current situation, anticipating future needs, and creating a clear plan of action.
Resource Identification: We offer targeted referral services, connecting you directly with the Aitkin County CARE programs or other community and county resources that match your needs. These referrals can include assistance with in-home services, transportation, nutritional support, or caregiver respite.
Information and Guidance: Get clear, reliable information on available services that can assist the older adult in maintaining their health, safety, and quality of life at home.
Care Consulting empowers both the care receiver and the caregiver by cutting through the complexity of navigating long-term care options. It's the first step toward securing a sustainable and comfortable aging plan.

Advance Care Planning Service
Planning ahead ensures that your choices are honored – not left to others in a time of crisis. Important questions to consider include:

• Is your directive on file with your healthcare provider?
• Have you documented preferences such as CPR, Breathing Support, Pain Management, Feeding/Fluid Support, Organ Donation, or who has legal authority to make decisions for you?
Ann Gile can help guide you through this process in the comfort of your own home. The one-time fee is $40, which includes notarized documents and assistance with uploading documents.
